Please read the following restrictions before continuing

For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

Cancellation Policy:
If you need to cancel your appointment, please do so at least 24 hours in advance to avoid a cancellation penalty.

New Patients:
If this is your first visit to our clinic or you have not been seen in the past three years, please select “New Patient” when scheduling.

Appointment Availability:
If you are unable to find the appointment time you’re looking for, please call us at the number listed at the top of this page—we’ll be happy to assist you.

Medical Aesthetic Appointments:
Most medical aesthetic treatments require an initial consultation, and same-day treatment is not always guaranteed.

Referral and Insurance Authorization:
If your appointment requires a valid referral or authorization from your insurance provider (such as an HMO, VA or Medicare Advantage Plan ), please contact us directly before scheduling.
