For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

If you have an HMO healthcare plan, the clinic MUST have valid authorization on file before scheduling your appointment. It is the patient’s responsibility to provide a referral or appointment may not be honored.

Please note, the following providers have age restrictions:

  • Sean Condon, MD does not see patients under the age of 13.
  • Dannie Change, MD does not see patients under the age of 12.
  • Breanne Hesseltine, PA-C does not see patients under the age of 6.
  • Jacqueline Capaldi, NP does not see patients under the age of 6.
  • Binh Ngo, MD does not see patients under the age of 5.
  • Christine Cole, MD does not see patients under the age of 2.
