For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
We accept the majority of the insurance carriers in our area, but please call your insurance provider to confirm we are in network with your plan.
To schedule a self-pay or to speak with someone in the office, please call 412-595-0430, Option 2.
If you have a medicaid insurance plan, to schedule an appointment, please call 412-595-0430, after ensuring that we are in-network with your plan.
If you insurance requires a referral, please obtain that referral before self-scheduling.
