Please read the following restrictions before continuing

For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

If you have an HMO or VA Community healthcare plan, please call 920-968-1790, Option 1 to schedule your appointment.

Please call 920-968-1790, Option 1, to schedule the following appointments:

  • Accutane
  • Biopsy
  • Mole removal
  • Skin tag removal
  • Stitch/suture removal

If you are an existing patient and need to schedule a surgery, please call 920-968-1790, Option 2

Cancellation Policy: If you need to cancel your appointment, please do so at least 24 hours prior to your scheduled time to avoid penalty.
