Pathology materials request

Step 1: Please read this carefully:

• Continuation of care: As a courtesy, original slides, unstained slides, or blocks (for molecular / prognostic testing) may be sent out to another healthcare facility upon request for the purpose of continuation of patient care.
• Patient Charges / Insurance Verification: It is the responsibility of the requester and/or receiving facility to communicate to the patient any applicable charges for their intended services, as well as to verify insurance coverage eligibility for such services.
• Turn around Time to receive materials: Materials will be shipped within 7 business days of eligible request. If materials are not available, we will notify the requester via phone or email.
• Shipping costs: Shipping costs will be the responsibility of the requester or receiving facility. Please have your FedEx or UPS shipping account number (or label for upload) available when submitting requests.
• Available Materials: Forefront retains tissue blocks for 2 years and slides for 10 years per CLIA requirements. Older materials are not available to be released.
• Return of Materials to Forefront: Materials are expected to be returned within 30 days from receipt along with a copy of any diagnostic reports generated.
• Research: Materials will not be released for research purposes.
• Clinical Trials: If requesting materials for a clinical trial, please have the patient sign our release to submit with your request: Authorize release form (the form is in the attachments)
• Patient personal requests: We cannot release slides directly to patients / non-healthcare providers due to associated risks. As an alternative, representative photos of slides may be provided.
