Minors (under 18) must have parental or legal guardian consent to schedule appointments. Additionally, a parent or legal guardian must accompany minors during the initial visit and, in some cases, subsequent visits to provide informed consent.

We do not accept Medicaid.

Cancellation Policy: If you need to cancel your appointment, please do so at least 24 hours in advance to avoid a cancellation penalty.
