Please read the following guidelines before continuing
For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
If you have insurance with the VA, please call 513-845-8120, Option 1 to schedule your appointment. A current referral must be on file.
Please call 513-845-8120, Option 1, to schedule the following appointments:
- Nurse visit
- Cosmetic procedure including BOTOX®, BBL-laser, or chemical peel
- New patient
