If you have a VA Community Healthcare plan, please call 812-401-8999 to schedule your appointment.
At this time, we do not accept Medicaid insurance unless secondary to Medicare. If you have any questions about insurance, please call our office.
Please be aware that there is a $75 fee for any cosmetic consultation. If cosmetic services are provided same day, this fee will go toward those services. To schedule your cosmetic appointment please call our office at 812-401-8999, or complete the cosmetic appointment request form.
