Age-Based Restrictions
Minors (under 18) must have parental or legal guardian consent to schedule appointments. Additionally, a parent or legal guardian must accompany minors during the initial visit and, in some cases, subsequent visits to provide informed consent.

• If you need a language interpreter, please call the clinic to schedule an appointment.
• For same-day appointment availability, please contact the clinic.
• Cancellation Policy: If you need to cancel your appointment, please do so at least 24 hours in advance to avoid a cancellation penalty.
