Thank you for requesting an appointment with us. Our office will contact you to schedule your appointment. We strive to reply within 3 business days from time of your submission.
Please read the following guidelines before continuing
We want to ensure your appointment is scheduled correctly to properly address your concerns. The following appointments cannot be made online. Please call your preferred Fairfax office to be seen for:
- Any procedures (lesion/mole removal, biopsy, excision)
- Phototherapy (UVB)
- Biologic or Accutane appointments
Please note, we currently do not accept any Medicaid plans. Most HMO plans require a referral from your primary care physician.
A $150 cosmetic fee is required for all scheduled cosmetic appointments with Dr. Freidrich Anselmo and Dr. Michele Magnino. Should the cosmetic service be performed on the same day, the fee will be credited toward the cost of the procedure.
