Please read the following restrictions before continuing

For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

Please note that we do not currently accept new patients with Medicaid, Wellcare, or McLaren Health insurance.

If you have a healthcare plan in the Blue Care Network, a referral from your Primary Care Physician must be faxed to our office prior to your appointment at 586-263-5183.

If you are a patient with a Total Healthcare plan and wish to schedule an appointment, please call 586-263-3130.
