Please read the following guidelines before continuing
For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
If you have an HMO, your insurance may require that you get a referral from your primary care physician prior to your appointment at our office. If you have questions about insurance, please call our office to schedule an appointment.
Please note that Dr. Grekin only sees patients 1 year and older.
