For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

Please note, this office is currently not accepting Medicaid patients. If you have an HMO plan, your insurance may require you get a referral from your primary care physician prior to your appointment at our office. If you have questions regarding insurance, please call 810-982-9527, Option 1, to schedule your appointment.
