For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

Accutane Visits
Please call the clinic to schedule Accutane appointments.

If you have Medicaid or an insurance plan with the VA, please call 812-282-8622, Option 1, to schedule your appointment. Please note that for a VA plan, you must have a current referral on file.

Referral Requirements: Some insurance providers mandate a referral from a primary care physician before scheduling. Without this referral, patients may need to assume financial responsibility for the visit.
