Please read the following restrictions before continuing

For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

If you have Medicaid or an insurance plan with the VA, please call 812-282-8622, Option 1, to schedule your appointment. Please note that for a VA plan, you must have a current referral on file.

Please call 812-282-8622, Option 1, for the following appointment needs:

  • All new patient appointments
  • To schedule a cosmetic procedure with Dani Haas or Dr. Moye, or complete the cosmetic appointment request form. Please note that cosmetic procedures are limited in this office, but we may be able to accommodate you in our downtown Louisville location.
