Terms and Conditions for Online Credit Card Payments
All payments from Forefront Dermatology invoices using the online credit card payment tool are subject to the following conditions.
Forefront Dermatology ACCEPTS the following credit cards:
By entering your credit card information:
You are stating that you are an authorized user of the credit card and that the associated information (account holder name, account number, billing address, etc.) is accurate.
You authorize Forefront Dermatology to charge the amount you have requested to your credit card.
You authorize Forefront Dermatology to return to your credit card any funds due to you if an overpayment exists.
There is no additional fee associated with using the Forefront Dermatology online payment tool.
Due to the way transactions are processed by our external processor, there may be a delay of 1-3 days in updating your account in the Forefront Dermatology records.
Payments are debited to Forefront Dermatology. Each credit card number is secure and protected with a high level of encryption when transmitted over the Internet. Forefront Dermatology does not store your credit card number after the payment has been submitted.
The online payment tool will allow for 1 credit card payment per day for security purposes. Therefore, if you would like to make payments on multiple accounts on a single day with one credit card we recommend that you add all of the payment amounts together and make one lump sum payment for all accounts. If you make one payment for multiple accounts, please be sure to include each patient account number and name that should have funds posted to their account.
Any prompt payment discounts that are applicable will be applied after your credit card payment has been posted to your Forefront Dermatology account.
When you complete and submit the online payment form, funds will be deducted from your credit card within one business day if the transaction is approved.
Upon submitting your credit card payment with the online payment tool, you will be notified immediately upon submitting the payment if the transaction has been approved or declined.
If the transaction is approved, you will receive a receipt on your screen for the payment and you will receive an e-mail notification that the payment has been approved. You will be provided with an authorization number on the receipt that is generated when the payment is approved; this authorization number will verify your payment if for some reason the payment is not posted to your Forefront Dermatology account.
If the transaction is declined, you can try submitting the payment one additional time and if the transaction still declines, we recommend you contact your credit card provider for details.
Forefront Dermatology will treat a payment from a debit card as if it were a credit card.
If after you submit your credit card payment via the online payment tool you feel that you made a payment in error, please contact our billing department and they will be able to assist you with any requests or questions you may have.
All information that you provide on the credit card payment forms will be handled in accordance with all federal regulations.
Forefront Dermatology's Terms and Conditions for online credit card payments are subject to change at any time. Each transaction shall be subject to the specific Terms and Conditions that were in place at the time of the transaction.
