For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment.  Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.

Please be aware that we do not accept Care Credit, or Trilogy MA at this time. If you have a plan with one of these providers and still wish to make an appointment, please call 414-355-2405, Option 1, to schedule your appointment.

  • You are a patient who has been seen by Dr. Berman at another location
  • If you have Medicaid insurance or Medicaid as secondary insurance
  • If you are a current patient and would like to schedule a UVB appointment
  • If you are a current or new patient wishing to schedule a cosmetic consultation or complete the cosmetic appointment request form
