Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
Does NOT accept Cigna Connect (from health exchange marketplace).
Does NOT accept: Medicaid (including Anthem HealthKeepers Plus and Optima Family Care).
Providers do not see patients under the age of 5.
