For patients scheduling who are under 18 years of age (19 in Alabama) please make sure you have permission from your parent or legal guardian to schedule this appointment. Your parent or legal guardian must accompany you on your initial visit and on certain subsequent visits to provide appropriate informed consent.
Please be aware that at this time, we do not accept Medicaid, Care Credit, or Workman’s Comp. If you have any questions about insurance, please call our office at 317-846-2396.

- Visits with Dr. Bailey Tayebi
- Cosmetic Visits (including Botox + Filler) or complete the cosmetic appointment request form
- Injections (including Xolair)
- Accutane
- XTRAC Laser Procedure
- Stitch/suture removal
- Photo Dynamic Therapy (PDT)
If you are in need of an injection, please call the office to schedule to ensure the medication is approved and available by your appointment.
